About Study Course

Objective

The course is designed for cardiology residents to acquire theoretical and practical knowledge, as well as practical skills on: 1) patient care before and after heart surgery, 2) monitoring and treatment of patients with hemodynamically unstable acute cardiovascular pathologies, as well as providing first aid in emergency situations.

Learning Outcomes

Knowledge

1.- understanding of patient evaluation before cardiac surgery. - patient care standard before and after cardiac surgery, - early and late complications and their treatment tactics. - specialized cardiovascular care: • Indications, limitations, complications. Interpretation of invasive and non-invasive hemodynamic measurements; • Indications, contraindications for the use of pharmacological agents used in circulatory support (fluids, inotropic agents and vasopressors). - principles of respiratory support: • Interpretation of arterial and venous blood gas samples; • Causes, prevention and treatment of respiratory failure; • Provision of airways in acute situations; • Principles of oxygen therapy and selection of oxygen supply devices; • Indications for the use of various invasive and non-invasive lung artificial ventilation methods (including the general principles of MPV); • Effect of artificial lung ventilation on the cardiovascular system (circulation); • Acute lung injury/ARDS (pathogenesis, diagnosis, prevention, principles of therapy); - Principles of infection prevention and treatment; • Selection of antibacterial drugs, indications, complications, interactions; • Sepsis, septic shock and SIRS.

Skills

1.- Be able to identify, assess and stabilize a hemodynamically unstable patient. - Be able to use medical equipment to identify early cardiovascular system disorders that require urgent action: • Creation of arterial and central venous access, pulmonary artery catheterization; • Measurements and interpretation of hemodynamic readings; • Selection and use of inotropic, antiarrhythmic and vasoactive medications, as well as liquids. - Ensuring the respiratory system: • Ability to identify early signs of acute respiratory insufficiency and respiratory failure; • Tracheal intubation; • Be able to obtain and interpret blood gas samples. (arterial, central, mixed venous blood). • Correct fluid, electrolyte, metabolic and glucose imbalances. - perform non-invasive and invasive monitoring and provision of vital functions of hemodynamically unstable and acute patients. - recognize early and late complications of heart surgery.

Competences

1.– the ability to prepare a patient for cardiac surgery, – the ability to recognize early and late complications of heart surgery and organize the treatment process, – to be able to provide adequate patient care after heart surgery. – The resident must be able to perform non-invasive and invasive monitoring and provision of vital functions of hemodynamically unstable and acute patients.
