About Study Course

Objective

To develop students’ knowledge and skills in chronic patient's care by providing competences for effective assessment of the patient’s health status, planning and implementation of therapeutic interventions, and for the management of multidisciplinary teams. The course will provide an in-depth understanding of the principles of chronic patient care and their application in clinical practice, developing both theoretical knowledge and practical skills.

Preliminary Knowledge

Aetiology of diseases, pathogenesis, clinical scene and treatment and care interventions, epidemiology, preventive measures, functional and laboratory diagnostics.

Learning Outcomes

Knowledge

1.As a result of the study course acquisition, students shall: 1. Acquires an in-depth understanding of disease aetiology, pathogenesis, clinical picture and treatment and care intervention options. 2. Knowledge of clinical medicine, treatment and action algorithms, guidelines, use evidence-based practices. 3. Navigates medicated treatment options and is able to prescribe, correct medicated therapy. 4. Able to prescribe and prescribe medicinal products and medical devices in the specific clinical situation, in compliance with the requirements of regulatory enactments.

Skills

1.As a result of the study course, students will learn: 1. The skills to assess a patient’s health status and analyse its dynamics. 2. Ability to plan and implement therapeutic interventions according to patient needs. 3. Skills in assessing patient health, drawing up conclusions and making clinical decisions about diagnoses. 4. Ability to diagnose chronic diseases and develop a treatment plan. 5. Ability to make decisions on the application of therapeutic interventions to patients with chronic diseases. 6. The skills to apply evidence-based behavioural algorithms and clinical guidelines. 7. Ability to plan and manage person-centred healthcare by promoting the involvement and responsibility of the patient and their family for their health. 8. Skills to organise and manage collaboration across multidisciplinary teams to ensure effective patient care.

Competences

1.Ability to apply information and communication technologies. Ability to make an autonomous decision on the conduct of therapeutic interventions by developing and applying evidence-based action algorithms. Ability to interpret and provide information comprehensible to the individual, group and community about the results of the examinations conducted and the necessary follow-up actions. Ability to make clinical decisions within the framework of evidence-based Nursing practice using research methodology and terminology. Ability to prescribe and prescribe medicinal products and medical devices in the specific clinical situation in compliance with the requirements of regulatory enactments. Ability to evaluate treatment indications, contraindications, and side effects. It was possible to evaluate, select, and apply technological solutions in patient care. Ability to assess and identify the benefits and risks of the use of medical technologies and devices, their impact on the human body. Ability to plan and implement the process of educating the individual, group and community on health issues, medication and disease prevention through teaching and learning strategies and to evaluate its effectiveness. Ability to plan and implement the patient’s education process on health issues, medications and disease prevention, taking into account the individual needs of the patient. Ability to monitor and manage the treatment and care of various wounds using appropriate therapeutic interventions and medical technologies. Ability to perform invasive and non-invasive respiratory diarrhoea and respiratory support using medical technologies. Ability to perform invasive and non-invasive fluid and nutrition admissions support using medical technologies. Ability to perform urogenital system care and stoma replacement for the most common urogenital abnormalities. Ability to monitor and manage the treatment and care of the most common endocrine disease cases. Ability to assess the effectiveness of care activities. Ability to develop and implement a strategy based on research outcomes in healthcare. Ability to develop professional activities, use information and communication technologies in a convincing and secure manner in the performance of the terms of reference.
